What does rehabilitation case management involve?
Corporé’s case management team begin by expertly assessing the injured party’s full rehabilitation needs in order to gain a full understanding of their personal goals.
All of our case managers are qualified professionals with clinical, rehabilitation and case management experience. Their key disciplines include Occupational Therapy, Nursing, Physiotherapy and Psychological Therapy. On top of this, they are specialist experts in Acquired Brain Injury, Spinal Cord Injury and Amputation to name a few. Their overriding dedication is to return a client as close to pre-injury state as quickly and effectively as possible.
Appropriate treatment at the earliest opportunity.
Our case managers undertake a detailed Initial Needs Assessment to identify all the needs of the client and any potential barriers to recovery. They then create a bespoke programme to coordinate outcome focused rehabilitation that ensures the injured party is engaged in appropriate care pathways, and receives treatment at the earliest opportunity through both the NHS or private providers.

Virtual Case management
Quality case management is crucial to ensuring that the rehabilitation goals of an injured party are met. To this end, following a detailed Initial Assessment, our team identifies and defines the injured party’s rehabilitation goals through telephone or video call examinations.
Our virtual case managers then coordinate effective treatments based on these goals. This ensures the right treatment and the highest standards of care are maintained, as well as enabling our team to monitor progress wherever the injured party is located.
Field case management
Corporé’s multi-trauma and catastrophic case managers are available for individuals suffering multiple traumas or injuries, or substantial musculoskeletal injury and engage with the injured party on a face-to-face basis. They focus on facilitating timely, outcome focussed intervention and returning the injured person back to the most optimal way of life.
